Guidelines for completing the application for enrolment form
Initial instructions
This guide helps you to complete the Application for Enrolment form and/or Unit enrolment form.
- Future students: Please follow the instructions in this guide to help you when filling in your form/s.
- Current students: Please refer only to the section/s for which you require assistance when filling in your form/s.
Where information is not available in this guide, please refer to the ALC handbook or relevant website locations.
How to complete the form
The forms are designed to be completed electronically. We suggest you
- Save to your computer
- Tab [or scroll] to each question and type in your response
- Tab [or scroll] to each box and press the space bar or click with your mouse and a [X] will appear. To remove the [X] press on the space bar or click with your mouse again

Personal details section
Personal details
- CHESSN no
The Commonwealth Higher Education Student Support Number [CHESSN] is a unique identifier used by all higher education providers in communication with the Australian Government. Students who are recipients of Commonwealth assistance will need to use a CHESSN to access their records via myUniAssist. MyUniAssist is a web-based Higher Education Information Management System [HEIMS], developed by the Commonwealth Government to assist in the management of fee arrangements and allows students to monitor their:
- Student Learning Entitlement [SLE]
- Higher Education Loan Program [HELP] assistance
- Commonwealth Learning Scholarship
How do I get a CHESSN?
The CHESSN is included on the Commonwealth Assistance Notice [CAN] which is provided by ALC to students electronically after the census date for each study period.How long do I need a CHESSN?
You will only ever be allocated the one CHESSN for the rest of your academic life. It will continue to be used after your academic award is completed for monitoring Commonwealth assistance and allocating lifelong SLE.Informed consent
In the application for enrolment form you are asked to consent to personal, identifying information being provided to the Department of Education, Employment and Workplace Relations [DEEWR] and the Australian Taxation Office [ATO] for the allocation of a CHESSN and the management of your Commonwealth assistance.Your personal details will be used to verify your identity and to ensure that you are not issued with a duplicate CHESSN.

Further information about myUniAssist and CHESSNs can be obtained from the 'going to uni' website at: www.goingtouni.gov.au.- Names
Ensure you provide any previous surname[s] or family name[s] as well as your current name and that your given names are consistent with your official records eg Katherine not Kate. This information is particularly important if you have prior studies under a previous name.
If you wish to be called by a name different from that listed as your given name please enter it next to ‘Preferred name’.

Please note that:- only Australian citizens and students with a permanent humanitarian visa can apply for FEE-HELP.
- certified documentary evidence is required for those who are not an Australian citizen.

Please indicate the language you speak at home. If you are a temporary resident, enter the language that is spoken in your country of origin.

Please indicate any special needs [disability/impairment, long term medical condition or English language need] which may impact on your studies and any assistance/special consideration or reasonable adjustments you may require.

Education level of your parents or guardians
For statistical purposes the Australian Government Department of Employment, Education and Workplace Relations requires ALC to collect information about the highest level of education completed by your parents or guardians.
Enter the answers about the people [up to two] who, during all [or most] of your school years, were your parents or guardians. If you have more than two, answer about those you have spent the most time with.

- If you are a teacher, or applying for or enrolled in a teaching course: Select School of Educational Theology [SET]
- If you are a pastor or wishing to train as a pastor in the LCA: Select School of Pastoral Theology [SPT]
- If you are a nurse, lay worker, chaplain, social worker etc or studying in these areas, or if you wish to study theology: Select School of Theological Studies [STS]
- Credit transfer and recognition of prior learning [RPL]
Credit transfer is a process which allows you to seek recognition for prior studies and/or life skills/experience. If you are eligible for credit transfer you may be able to complete your studies within a shorter length of time. Course completion details

If you plan to complete your course of study by the end of 2010:
Select Yes to I am planning to graduate from ALC in 2010.If you are not planning to complete your course of study by the end of 2010 but you do intend to complete the course in which you are currently enrolled:
Select I am enrolled in a full ALC course [not Lutheran Strand students enrolled in DipTh(Ed)] and enter the year you plan to complete the course.If you do not intend to complete the course you are currently enrolled in:
Enter the year you plan to complete your final unit of study with ALC.
Then select either
I am enrolled in selected unit/s of a course only and intend to graduate from another institution
or
I am enrolled in selected unit/s only and do not intend to complete a course at ALC.Part time students
If you are part-time and intend to complete the entire course, either predict the total time you plan to complete the course, otherwise we suggest you double the nominal length of the course you choose.- For students in the Lutheran Strand

Undergraduate courses
For further information contact the relevant Head of School or refer to the Schools at ALC on the website [www.alc.edu.au/schools/] or the ALC Handbook.
Postgraduate courses
For further information contact the relevant Head of School [coursework degrees] and/or the Postgraduate Research Coordinator [research degrees] or refer to the ALC Handbook.
Unit enrolment section
If the unit enrolment section does not appear in your Application for enrolment form, you will need to enrol in your unit/s via the appropriate unit enrolment form and complete as per the following timeline:
- Future School of Pastoral Theology [SPT] students—after you have been approved to study
- Future overseas students [with a valid student visa or pending application]—in consultation with the appropriate Head of School during the orientation period
Once you have completed any unit enrolments for the year, use the appropriate unit enrolment form to enrol in further units.

The key lists the unit study modes offered through ALC. When you select a unit in any study period, only the study modes available for it are listed.
Only select the option of ‘other’ if the unit which you are required to complete for your course is not available by suitable study modes. If this is the case, please consult the Head of School to discuss how you can complete the unit.

Scroll down to locate the appropriate course and make your choice/s for the year [or individual study period] from the listed core and elective units.

- Workshops
Whenever you have selected ‘mixed’ study mode as your unit study option, you need to complete this section—enter the details of the unit, venue and dates.
Workshops scheduled for the year are listed in the 2010 workshop calendar [PDF].
Further workshops in units listed on the workshop calendar may be offered if a cohort of at least 6 students is available in any region. To indicate your interest in any workshop not currently scheduled, select the study mode option of ‘other’ for the pertinent unit on your application for enrolment form and contact the Head of School to discuss your needs.
- Audit units—not for credit [undergraduate courses only]
Enter unit details as required.
Note: Students may audit an undergraduate unit at the discretion of the lecturer, and with the approval of the Head of School. [Regular class attendance is required.]

If your school has agreed to pay your tuition fees [in full or part] it is your responsibility to ensure that this is done.

Please forward that to the person responsible for making the payment and ensure that the full tuition fee is received by ALC prior to the commencement of the study period for which the unit payment is required.
